    Fillers are implants (fillers, gels) based mainly on hyaluronic acid. And they are used and indicated for correction of the face shape and restoration of lost volumes. That is, as with breast implantation, when there is no necessary volume, implants are “inserted” and the breasts are enlarged. Also, fillers for the face, where there is a tissue deficiency – nasolabial folds, nasolacrimal groove, chin, cheekbones, etc.

At the first stage, it is necessary to stimulate the synthesis of hyaluronic acid and collagen production to improve the quality of the skin – hydration, improvement of turgor and color of the skin.     Botulinum therapy or, as it is now popularly called, “Botox” is the introduction of a medicinal preparation of botulinum toxin to “relax” the facial muscles. This procedure does not help to restore lost volume. It works by relaxing some and stimulating others, resulting in a redistribution of facial tissues and relaxation of wrinkles. But this does not always happen and not in everyone. Much depends on the facial activity and condition of the facial muscles. And here, too, clear indications are needed.     Botulinum therapy uses different toxins from different manufacturers, mainly France, Germany, Korea. They are all practically the same in their action and duration of action. How long the drug will “stand” does not depend on the manufacturer, but on the individual characteristics of the patient. Manufacturers of drugs in the instructions for use indicate periods from 3 to 6 months. However, in some patients the effect of the drug can be observed for a year, and in someone – no more than 2 months. These are already individual characteristics of the patient.

    Over the past 2 years, against the backdrop of coronavirus infection and vaccination, we have seen that the effect of toxins has significantly decreased, and this is primarily due to the activity and tension of our immune system. After all, it is the immune system that inactivates the toxin that we introduce into the body. The effect of the toxin is realized in the first 30 minutes after its introduction. Within 2-3 weeks, all muscles completely relax. And then a gradual restoration of neuromuscular transmission in the muscle fiber begins. And the duration of the toxin’s action depends on the speed of this restoration, on the level of biochemical processes in a particular organism.

    If a person is actively involved in sports, he has a high level of metabolic processes, then the restoration of muscle activity occurs quite quickly. That is, the effect of the toxin does not last as long as we would like. The same with fillers. When we introduce a foreign substance into the body, our immune system will do everything to remove it from the body as quickly as possible. The so-called “hyaluronic acid eaters” accelerate the absorption of the filler. Thus, the rate of absorption depends not so much on the manufacturer or the quality of the drug, but on the individual characteristics of the patient.

    Which fillers are best for the face?

    The choice of the drug is made only by a doctor – cosmetologist. Just as a patient cannot prescribe a drug to lower blood pressure, or for stomach pain, or antibiotics. Only a doctor at the appointment, having assessed the condition of your skin, the presence of contraindications, the result that you want, can determine which filler or botulinum toxin should be injected specifically for you.

    It is important to remember that only a filler that has state registration – a registration certificate – can be introduced into the body. Such procedures should be carried out only in medical organizations that have a license for medical care. Not all beauty salons and hairdressers have a license, and specialists who work in rented offices or at home do not have permission for private practice. And the risk of such specialists using uncertified and “gray” drugs is very high.
